Required State E-Waste Form
The California E-Waste Recycling Initiative is restricted to products used in California. A critical requirement by the State of California is for the approved collector to document the California source of electronic devices (i.e. full name, street address, city, zip code). To comply with this mandate EWM/ACQ Office requires a CEW Collection Log (e-waste form) to be submitted for drop off of any e-waste items on our ACCEPTED E-WASTE list. Click here to see accepted and not accepted e-waste items.
